If you submit any documents (copies or original documents, if requested) in a foreign language, you must include a full English translation along with a certification from the translator verifying that the translation is complete and accurate, and that the translator is competent to translate from the foreign language into English.

A police report or a sworn statement if you are applying for replacement of a lost, stolen, or destroyed document;

Evidence of a USCIS typographical or clerical error (if applicable);

Evidence of your legal name change (if applicable);

Evidence of your legal date of birth change (if applicable);

Evidence of your biological sex at birth (if applicable);

Evidence of your marital status change (if applicable); and

A copy of your original naturalization certificate if applying for special certificate of naturalization.
