Initial Form G-28

File Form G-28 with the related application, petition, motion, or appeal (in your USCIS online account or by mail).

Online: To file a form online on behalf of your client, along with Form G-28, you must first create a USCIS online account. By Mail: If you submit Form G-28 by mail, review our Tips for Filing Forms by Mail page for information on how to ensure we will accept your form.

Subsequent Form G-28

Online: You can submit subsequent Forms G-28 online for certain pending forms. In your USCIS representative online account, navigate to File a Form, and choose Form G-28 in the dropdown.

By Mail: If you are mailing a subsequent Form G-28 related to the same matter or form, mail it to the office where your case is pending, or to the return address of the last notice you received.

Filing Tips: If you do not properly complete your Form G-28, we will not accept it. This means we will process the benefit request as if you did not submit Form G-28.

To make sure we accept your Form G-28, properly complete the form according to the form instructions. The most common mistake we see is that the form is not signed or does not have all required signatures. Follow these tips to avoid other common mistakes:

Include the client’s first and last name exactly as they appear on the related immigration form.

Sign your Form G-28 with a black or blue pen. We will not accept a stamped or typewritten name in place of a handwritten signature. However, we will consider a photocopied, faxed, or scanned copy of the original handwritten signature valid for filing purposes. The photocopy, fax, or scan must be of the original document containing the handwritten, ink signature.

You and your legal representative must sign.

When signing, remember to tell us your preferences for where you would like us to send your notices and secure documents (for example, to you or to your attorney or accredited representative).

Make sure the information is clear on the form.

If you complete the form by hand, use a black pen and write inside the spaces provided;

Never use highlighters or correction fluid; and

Never gray out any part of the form.
